Center for Health Promotion
The Center for Health Promotion (CHP) partners with local community organizations, health systems, local and tribal public health, and other partners to prevent chronic conditions. We work to improve the health and wellness of those living with chronic conditions, with an emphasis on those experiencing the greatest health disparities.
CHP:
- Analyzes and reports data on preventable chronic diseases.
- Supports improvements in policies and systems of care that promote health.
- Empowers health care practitioners and individuals with tools, training, and support to prevent and manage chronic conditions.
- Partners with local community organizations and local and tribal health agencies to promote chronic disease prevention.
