Reporting

Hepatitis B is a reportable disease. In order to prevent perinatal transmission of hepatitis B to newborns, all hepatitis B-positive pregnant people must be reported to the Minnesota Department of Health.

Cases can be reported by completing the appropriate form below and faxing it to 1-800-334-4931.

• Perinatal Hepatitis B Pregnancy Report (PDF)
  Clinics should report hepatitis B-positive pregnant people within one working day of the knowledge of the pregnancy.
• Perinatal Hepatitis B Birth Report (PDF)
  Hospitals should report infants born to hepatitis B-positive people within 24 hours of birth.

Reporting Hepatitis
